MANITOWOC — Bellin Health Family Medical Center-Manitowoc will open Monday in the building formerly occupied by Copps Food Center.

Dr. Chris Kordiyak, a family medicine physician in Manitowoc since 1991, will move to the remodeled clinic building at 3415 Custer St., Suite D. Dr. Michael Hoffman will join Kordiyak at the clinic next year.

Bellin Health Family Medical Center-Manitowoc offers family medicine, including: 24/7 appointment scheduling, same day appointments, onsite bone density testing, lab services and more. The Manitowoc clinic accepts all major insurance plans, including United Health Care.

The new clinic will serve as an entryway to Bellin's other health service areas, including heart and cardiovascular care, oncology, orthopedics and robotic-assisted surgery, to name a few.

Bellin first entered the Manitowoc area in 2007 with its FastCare retail health clinic. This is the health system's first primary care clinic in Manitowoc.
